If you have made it all the way through the questionnaire and become stuck please make sure that all of your information is entered correctly.
Common errors listed below with how to fix them. Look for green check marks.
- City and Zip code do not match.
- Enter zip code first and city will fill itself in.
- Missing Fields
- Make sure that all fields on the address page are filled in.
- Phone number incorrectly entered (no hyphens)
- Example: 8009833887 NOT 800-983-3887
